Check with the OPP in your jurisdication for full details regarding the forms that they require. Bring these to your departmental office for signature. We can no longer provide the forms electronically as the OPP require "orginal signatures" on the documentation. Please note: Due to volume, you may have to leave these and pick them up at a later date.
In order for Hamilton Police Services to release or confirm confidential information, they require proper identification to verify your identity. One of the two required pieces must be photo ID.
Any two of the following pieces of identification are acceptable.
- Valid Driver’s Licence (with photo)
- Citizenship Documents
- Birth Certificate
- Immigration Documents
- Marriage Certificate
- Ontario Age of Majority Card
- Baptismal Certificate
- Student Card (with photo)
- Current Passport
- Native Status Card
- Valid Firearms Licence
- Employment I.D. (with photo)
- Vehicle Insurance or Ownership
- Health Card (with photo)
** Utility bills or Government mail with your name and current address can be accepted if accompanied by photo I.D. and confirmation of date of birth **
