Checklist for Reporting Accidents/Incidents

In case of accident/incident:
checkbox Promptly obtain first aid treatment. For any injury requiring health care, employee shall obtain from their Supervisor a completed Treatment memorandum (Form 156) to take to the doctor or hospital.
checkbox Employee/Supervisor together initiate, complete and sign Accident/incident Form 0361. Supervisor distributes Form 0361 as per routing on bottom of form.
checkbox If employee requires health care and/or loses time from work, Supervisor must inform the Occupational Health and Safety Co-ordinator (ext. 2225) immediately.
checkbox If lost time accident, critical injury or hazardous situation exists, Supervisor must inform Occupational Health and Safety Co-ordinator (ext. 2225) immediately.
checkbox If lost time accident, employee is to keep Supervisor informed of status and supervisor is to advise Occupational Health and Safety Co-ordinator (ext. 2225) immediately.
checkbox Prior to return to work, the employee must provide to their Supervisor a Return to Work Form completed by a doctor. The Supervisor will forward the written clearance to Human Resources and the Occupational Health and Safety Co-ordinator.
