Checklist for Reporting Accidents/Incidents

In case of accident/incident:

checkbox Promptly obtain first aid treatment and/or emergency medical attention as necessary.

Report the injury to your manager.

checkbox Manager and Employee review incident, identify preventive measures, sign and distribute Accident/Incident Form.
checkbox If employee requires health care and/or loses time from work, Manager must inform the Occupational Health and Safety Consultant (ext. 2225) immediately.
checkbox If lost time accident, critical injury or hazardous situation exists, Manager must inform Occupational Health and Safety Consultant (ext. 2225) immediately.
checkbox Employee must provide a completed Functional Abilities Form if returning to work from a lost time injury and when requiring modified duties.
