Tourism Employer Participation Form

Tourism Students are available for placement April 8-19, 2024, to participate in the Work Experience Partnership, please complete this form.

If you have been approached by a specific student and wish to provide a placement for them, please be sure to enter their name on the form. If there is no student name indicated, the placement will be open to all students. 

*To host a student employers must sign "The Mohawk College Student Placement Agreement" if you haven't submitted this form previously it will be emailed to you after you submit this form.

Employer Information

WSIB Coverage Information

Please indicate the form of WSIB coverage that you have at your company/organization. The Ministry will be covering the student for the unpaid placement by the same method which you cover your regular employees (either WSIB or Private Insurance).

By selecting yes below - you understand, the Placement Employer hereby agrees to immediately report any workplace injury, illness or disease involving a student on an unpaid work placement to Mohawk College. Where the Placement Employer is covered by WSIB, they will also complete a Letter of Authorization to Represent the Placement Employer that allows Mohawk College to represent the Placement Employer on matters related to the WSIB claim.

The Placement Employer agrees that it will provide the Student Trainee with health and safety training and supervision to protect them from health and safety hazards that may be encountered at the Placement Employer's workplace and assist them with completion of the "Student Work Placement Safety Checklist."
