OSAP Reviews and Appeals

 A student may request that their OSAP assessment or bursary eligibility be reviewed if they have a special situation which is not recognized in the standard OSAP or bursary assessment of financial need.

The following appeal outlines and forms will assist you in determining if your situation falls within an allowable review.  If you have determined that your situation falls within one or more of the allowable reviews below, print off the cover page, attach all letters and supporting documentation and submit your package to the Financial Assistance Office for review.

All letters must be legible, signed and dated and your student number must be clearly noted.  Letters from third parties must include contact information for the person who is corroborating the situation and making a recommendation. 

The deadline date for submission of a review to the Financial Assistance Office (FAO) or Ministry of Training, Colleges & Universities, including requests for reconsideration, is 40 days before the end of your study period All required supporting documentation must be submitted by the deadline.

Requesting a review of your full-time funding

Depending on your situation, you can ask for a review of the financial aid you are eligible to receive.

Some examples include:

  • you have unanticipated financial difficulties
  • you’re unable to make your expected contributions
  • you’ve experienced a family breakdown since submitting your application
  • you’re not progressing academically due to circumstances beyond your control
  • you’ve declared bankruptcy

To start a review: you need to meet with your Financial Assistance Advisor.

Review request deadline

You need to submit a request for a review 40 days before the end of your study period.

Situations that do not qualify for a review:

  • you’ve received the maximum amount of aid
  • your parent(s) arbitrarily refuse to contribute to your education-related costs
  • your citizenship status
  • requests past the deadline

Income Appeals

Income appeals are used to request a review of your OSAP file based on exceptional circumstances regarding your income, your parent's income, or your spouse's income.

Request for Assessment on Parent's Estimated Income (2018-19) (PDF)
Use this form to request that your parent’s estimated 2018 income be used instead of their 2017 income in the assessment of your 2018-19 full-time application.

Parent(s) Income Verification: Canadian Non-Taxable and/or Foreign Income (2018-19) (PDF)
If your parent(s) income can’t be verified with the Canada Revenue Agency (CRA), they must complete this form. This includes signing the declaration and providing the required documents.

Request for Assessment on Spouse's Estimated Income (2018-19) (PDF)
Use this form to request that your spouse’s estimated 2018 income be used instead of their 2017 income in the assessment of your 2018-19 full-time application.

Request for Review: Student Fixed Contribution (2018-19) (PDF)
Use this form to request a review of your expected financial contribution used in the assessment of your 2018-19 OSAP Application for Full-time Students.

Adjust Income Contribution Due to Lack of Employment Form (PDF)
Use these forms to request a review of the expected income contribution from you and/or your spouse (if applicable) used in your assessment. This review will only apply to your OSAP assessment for federal loans and grants.

Significant Income Variances - Student Income Verification
The Ministry verifies student income as reported on the OSAP application against income information provided to Canada Revenue Agency (CRA) by the student. If the Ministry determines a significant variance between these income amounts, then the student is notified in writing of the duration of the resulting restriction.

Job Search History (PDF)
Complete this form, if you are appealing that you were unable to find full-time employment during your pre-study period.

Lower Estimated Income (PDF)
Your 2017-2018 OSAP entitlement is calculated using your parent’s/spouse’s 2016 income. If your parent’s/spouse’s income for 2017 will be a better indicator of the family’s current financial situation due to a non-recurring and major change, then you may ask for a reassessment using the estimated 2017 income.

Expense Appeals

Expense appeals are used to request a review of your OSAP file based on exceptional expenses that you have incurred during the pre-study or study period.

Adjust living allowance form (PDF)
Use this form if one of the following scenarios apply to you:

  • You're an independent student living with your parent(s) during your pre-study and/or study period and you pay them room and board or rent
  • You're a dependent student and don't live in your parent's home during your study period AND your parent(s) live within 30km of the University
  • You're a dependent student and did not live in your parent(s) home during your pre-study period because:
    • you were employed full-time in a different community than where your parent(s) live; or
    • there's an overcrowding situation in their home; or
    • there are psychological or physical health reasons preventing you from safely living there


Request for Review: Adjust Local Travel in Study Period (2018-19) (PDF)
Use this form to request a review of the local travel costs used in the assessment of your 2018-19 OSAP Application for Full-Time Students.

Adjust local travel in pre-study period form (PDF)
Use this form if you incurred travel costs during the pre-study period to obtain or maintain employment.

OSAP Request for an Exceptional Circumstances Review (PDF)
Use this form if you've incurred and paid exceptional expenses during your pre-study or study period.

Note: Many exceptional expense reviews apply only to your OSAP assessment for federal loan and grants. There may be no (or minimal) impact on your OSAP assessment for provincial loan and grants.

Family Breakdown

Family Breakdown Review Form (PDF)
This review allows an assessment without reference to one or both parent’s financial information in situations where there is a serious rift in the family and the student is estranged from one or both parents due to documented mental, physical, sexual, and/or emotional abuse or drug or alcohol addiction in the student’s family.

Residency Reviews

Complete the Residency review forms if you and/or your family have lived in Canada for less than 12 months, and Ontario has been the only province where you've resided. All forms must be completely filled in, and include original signatures.

Dependent Students:

Academic Probation

Academic Probation (PDF)
Use this form if you're restricted from receiving OSAP due to lack of academic progress and:

  • You've waited for the length of your OSAP academic restriction and would like to be reinstated back into the OSAP program, or,
  • Your OSAP academic restriction is still in effect but would like to appeal to have it lifted based on extenuating circumstances.

 

Credit Check Review Form

Use this form if your OSAP application was denied because of a failed credit check and you wish to have this decision reviewed.

Step Parent Appeal

Stepparent appeal form (PDF)
OSAP expects dependent students to provide parent and stepparent information on their OSAP application. This form can be used in some cases where a step parent does not provide direct or indirect support for the student and wishes to have their information removed from the OSAP assessment.

Federal/Provincial Restrictions

While the FAAs monitor, review and process academic progress issues, only the Ministry or the federal government/NSLSC can review or provide clearance for all other “student restriction” issues. These other “student restrictions” result from issues such as:

  • OSAP defaults;
  • Debt reduction:  
  • Debt Reduction in Repayment (DRR),
  • Repayment Assistance Plan (RAP) Stage 2, or
  • Repayment Assistance Plan for Persons with Permanent Disabilities (RAP-PD);
  • Significant income variances; and breach of contract.

The borrower may have any of the following:

  • Federal restrictions only;
  • Provincial restrictions only; or
  • Federal and provincial restrictions.
     


Federal and/or Provincial Restrictions (document)
Federal Restriction: Students who have federal restrictions (e.g., they have defaulted on their federal student assistance debts or have received Debt Reduction in Repayment, RAP Stage 2  or RAP-PD on federal loans) will have a restriction on their file.

Provincial Restriction: Students who have a provincial restriction on their file because they have defaulted on their provincial student assistance debt are not eligible for further provincial assistance until they rehabilitate their loan. Students who have received provincial debt reduction are not eligible for further provincial assistance until the provincial debt has been repaid in full.

Federal and Provincial Restriction: If the student has a federal and a provincial restriction issue, then upon receipt of proof of federal clearance from the student, the Ministry will release the current OSAP application for processing for federal funding only.

Taking Courses at Another Institution

Taking Courses at Another Institution (PDF)
Students must submit an OSAP application to the institution where they are paying the majority of their tuition and fees (i.e. taking the majority of their courses).  A student may be taking course(s) through another institution in addition to their courses at Mohawk.

Change Request

For students who need to report changes other than income, including their academic (course load), family or study period status and cancellation of their OSAP application.

For students who need to report changes if their study period and/or program changes during the 2017-18 academic year, as long as the school they are attending has not changed.

Use this form if you have already submitted a 2018-19 OSAP Application for Full-Time Students and have been advised by your financial aid office that you must provide parental information in order for your application to be considered.

It is also used to determine whether or not parental information is required as part of your 2018-19 OSAP Application for Full-Time Students.

This form is used to verify your status as a person with a disability for Ontario Student Assistance Program (OSAP) purposes.

Request for Grant-Funding Only

OSAP Request for Grant Only Funding (2018-19) (PDF)
Use this form to request only the grant(s) you are eligible to receive and to decline the Canada-Ontario Integrated Student Loan. In order to make this request, you must have already submitted a 2018-19 OSAP Application for Full-Time Students and have received an assessment of the amount of funding you are eligible to receive.
