POSITION SUMMARY:

Following Housing First principles, and as part of Mission Services’ Housing UP team, the Intensive Case Manager will provide ongoing support to individuals to secure permanent accommodations and maintain their housing. The Intensive Case Manager will work as a part of the Housing UP team and will engage regularly with service users in the community to provide the necessary support to achieve program and participant goals.

KEY RESPONSIBILITIES:

  • Assists in the engagement of prospective program participants who are identified through the Coordinated Access System
  • Assesses participant's needs for services to support their transition to housing and optimize their capacity to sustain housing using an acuity tool and other resources
  • Document and input information into service user files, manually and in computer databases, to ensure accurate service statistics and service user records
  • Develop, implement and monitor case plans and sustainability plans
  • Maintain regular, effective communication with social housing providers, administrators and private landlords, to ensure an integrated approach that will provide the best support possible for participants to maintain housing
  • Liaise with income support programs (i.e. OW, ODSP, CPP etc.) to assist in establishing an income source for participants
  • Assist each program participant with securing appropriate housing based on individual needs
  • Assist participants with their move into independent accommodations and provide neighborhood orientation
  • Provide enhanced support with maintaining safe housing and rapid re-housing where needed
  • Improve links with social, health, income and employment supports, as they relate to participant’s housing plans
  • Adhere to a strict policy of confidentiality
  • Work safely, identify any health and safety concerns and report to management, and follow Mission Services workplace health and safety policies and procedures at all times
  • Other duties as required to ensure the continuation of services
Skills and Experience Required

QUALIFICATIONS:

  • Successful candidates will hold at minimum a post-secondary degree or diploma in in social work, social service work, or a related field
  • Minimum one to three (1-3) years of related experience; three to five (3-5) years of experience is preferred 
  • Excellent computer, oral, and written communication skills
  • Experience working with individuals with mental health and addictions issues
  • Experience in case management techniques, use of, and connection to, community resources 
  • Ability to deal with confidential information and excellent interpersonal and diplomacy skills
  • Demonstrated ability to form helping relationships based on trust and empowerment 
  • Evidence as a highly motivated team player, with proven ability to take initiative and be self-directed
  • Committed to uphold the organization’s mission, values and standards

HOURS OF WORK:

  • 37.5 hours per week

    REQUIREMENTS:

  • Police Vulnerable Sector Check
  • Two-Step Tuberculosis Skin TestProof of double COVID-19 vaccination, or willingness to register to receive these vaccinations as soon as possible

    COMPENSATION: 
  • *Hourly wage starts at $24.13 per hour 
  • *Benefits after 3-months of continuous, active employment 
  • *Group RRSP employer matching up to 3% after one (1) year of continuous employment
