Clinical Placement Request Form - Continuing Education Health Sciences

NOTES:

• All Non-Academic Requirements (NARs) must be met before your placement begins.
• Complete this placement form on registering for your clinical course.
• Clinical placements may occur at locations that are not accessible by public transit. As such, students shall be expected to either have access to a car or the ability to supply their own transportation when attending clinical placement activity. Mohawk shall not be responsible for providing students with transportation to clinical placements.
• Registered students who have been convicted of an offence under the criminal code for which they have not been pardoned may be denied the opportunity to enter clinical placement.
• Students may be required to complete additional onboarding requirements/clearance requirements required by clinical placement agencies.
• We cannot guarantee your preferred location. We will attempt to secure your preferred request, however this may not be possible at times.
• Once a clinical has been secured, you will be contacted by the Clinical Area of Mohawk College with details on your clinical placement location.
