Registration Instructions

STEP 1:  To complete each section of the online registration menu. All information is kept confidential.  If you would prefer to submit a fillable copy of the Confidential Intake Form along with your documentation instead of using the online registration option, contact the Accessible Learning Services office at your campus (contact information below).  For more information about appropriate disability documentation, please refer to the disability documentation guide.

STEP 2: Book an appointment with Accessible Learning Services by contacting a Triage Officer at your campus (contact information below).  Once you complete the online registration, a Triage Officer from Accessible Learning Services will contact you with instructions on how to book an appointment using the preferred method of contact you have identified on the confidential intake form.

STEP 3: Attend your booked Accessible Learning Services appointment at the appropriate campus. A reminder is sent to your Mohawk email address 24 hours before your appointment.
