Please indicate the name of the faculty member for your section. Missing or incorrect information will result in your test request being delayed.
Unless approved by your instructor, you MUST schedule your test on the same date and time your class is writing.
All testing must be completed by 8:00pm. With your extra time, if your test ends after 8:00pm, please be sure to adjust your start time accordingly.
If you are scheduling your test on a different date or time, your test MUST be completed by 4:30pm.

Please list the testing accommodations required for your test (eg. Memory Aide, Breaks for Medical Reasons, etc). Please ensure these accommodations are on your Accommodation Letter.
