Please complete this form to help your Field Placement Specialist choose a Field Placement that best meets your needs.

Placement 1: Must be at an elementary school level with either the Public or Separate board.

Placement 2: Typically a High School. It may be the case that you are in a Specialized setting/classroom/school for this placement.You will chose this in your second semester.

Placement 3: Specialized setting/classroom/school, or in a High School, or in any setting if all requirements have been met in previous placements. You will chose this in your second semester.

Please note! There are specific placement requirements which must be met in order to graduate. Failure to meet these outcomes may result in your ineligibility to graduate. 

Field placements will not be assigned until this form has been submitted.

Do not contact any school to arrange your own placement. Doing so will disqualify that school as a placement choice.
