What do I bring? Suggested items include:

  • Bank account statements
  • A list of your expenses and income
  • Credit report
  • Debt information (i.e. line of credit, OSAP, credit card statements)

I hereby authorize a representative of Mohawk College to release and/or exchange relevant information including academic and/or financial records with any representative of the Ministry of Training, Colleges and Universities, or others as required. In consideration for services provided to me by The Mohawk College of Applied Arts and Technology (“Mohawk”), and in particular relating to my activities in the Mo’Money Program (“Program”), I hereby and forever discharge Mohawk and all of its affiliates, subsidiaries, predecessors and present and former governors, officers, directors, employees, servants and agents from any and all actions, causes of actions, suits, debts, demands, covenants, complaints, contracts, claims, applications, damages, liabilities, statutory rights and obligations representations, warranties and torts (both intentional and unintentional) whether presently known or unknown, which I may have or may hereafter have against Mohawk for any damages arising out of or in any way related to my enrollment and instruction as a participant in the Program.
