This form is completed by the faculty member when a breach of the academic integrity policy has occurred. Use MyMohawk and MyCanvas to help complete all sections of the form.  Upon submission this form is shared with the student, the Registrar’s Office and the Academic Department. Student Information

Use the College's systems of record (ie MyMohawk, FASTsuite and MyCanvas, etc.) to help complete the student's personal details, being mindful of the students preferred or lived name. 
Note: This is sensitive personal information, be sure you have indicated the correct MyMohawk email address for the recipient.
